Kindergarten Registration Docs

All incoming Kidergarten students MUST have a completed Universal Health Record (completed within one year of the first day of school), Health Hisotry and up to date immunizations in order to start school. If your child's birthday falls after the first day of school, please provide written documentation of a scheduled doctors appointment. If you have any questions, feel free to contact me.
